Elmtree Admission Form

Elmtree are here to support primary aged learners through to the transition into Year 11. This form allows us to assess the support we can offer and the opportunity for success alongside the current learners onsite. If we are unable to support the learner onsite as soon as possible outreach opportunities will be discussed.  

We are committed to making this process as smooth and efficient as possible, please provide any additional information you believe would benefit the learner achieving success with us.

Alongside this form, please provide any documents requested and complete the behaviour example [or provide your school equivalent if it provides the same information] all of the data will be used to ensure we can provide the best possible opportunity for the learner. 

Our objectives are for the learner to be able to successfully re-engage in mainstream education with any attainment gap between them and their age related/appropriate expectations being reduced.

Elmtree are unable to provide more than 17 hours per week and cannot be the sole provider of the learners education. Please provide details of the second provider if this will not be the host school identified earlier.

Any provision of learning provided by Elmtree is done so for a minimum period of 12 weeks [ideally 18] with review points of WK6, WK12 and WK18.

Please complete the document below, stage 3 is the peak of a crisis [as considered to be a crisis for the learner] we are looking for the physical/visual indicators as to where the learner is. You may not have seen some of the stages due to the experiences the learner is having outside of the classroom. The table is designed to give us an idea of what is currently being done to support the learner as well as what escalations in behaviour can look like. This information will enable us to ensure we are supporting all learners engaging with Elmtree.
